For First Time Applicant/s

• Authenticated Birth Certificate (NSO)
• Authenticated Marriage Certificate (for married woman only)
• CTC of Live Birth Certificate (Local Civil Registrar-LCR) – FORM No. 102
• 2 valid ID (SSS, Company, PRC, School, Drivers License ID)
• PERSONAL APPEARANCE TO DFA to be scheduled after the evaluation of documents.

For Minor Applicant/s

• Original passport (for renewal) and (new) fill-up the column provided in the application form for minors.
• For the applicant below 18 years old, either the parents should execute Affidavit consent & Special Power Of Attorney (SPA) if both parents are in abroad
• Attached Authenticated (Security Paper) from the National Statistics Office of:
• Child’s Birth Certificate
• Parent’s Marriage Contract
• Original and Photocopy of Passport of Mother/Father for traveling companion.

For Renewal Applicant/s

• Old passport (Original & photocopy)
• Authenticated Marriage Certificate from NSO (for married woman only who choose to adopt surname of his husband in her new passport)
• PERSONAL APPEARANCE TO DFA to be scheduled after the evaluation of documents.

Php 2,000.00 (70 working days until further notice by the dfa)

For School Documents (CHED/DFA) Requirements

• First endorsement from the School Registrar
• Certification of graduation
• Certified true copy of Transcript of Records (2 copies)
• Certified true copy of Diploma (2 copies)

 For Birth Certificate/Marriage Contract/Death Certificate/CENOMAR

• Authenticated BC/MC/DC/ Cenomar coming from the NSO
• Certified true copy of BC/MC/DC from the Local Civil Registrar
